We are very excited to welcome you and your child into the Church! For Catholics, the Sacrament of Baptism is the first step in a lifelong journey of commitment and discipleship. Whether we are baptized as infants or adults, Baptism is the Church's way of celebrating and enacting the embrace of God.

If you wish to have your child (6 years or younger) baptized at St. Bernadette, please fill out the form below. If the child you wish to baptize is 7 years old or older, please click here to learn more about the Rite of Christian Initiation for Children.

Please note the information on the form, including the Baptism date, is tentative. Both parents must complete the Baptism Preparation class prior to the official scheduling of the Baptism. Godparents are also strongly encouraged to attend the class.

Baptism classes are typically held on the first Wednesday of the month. Once we receive this request form, we will reach out to you to schedule the Prep class. Once the class is completed, we will reach out to you again to confirm the final Baptism date.
